Mental Health Assessment

The purpose of the assessment is to assess mental health status and how it is affecting the individual’s ability to perform daily activities, roles, values, attitudes, motivation, family and relationships. Functional Capacity Evaluation (FCE) is a standardized, quantifiable and intensive physical abilities examination whose purpose is to objectively measure a client’s current level of physical function and ability to perform functional or work-related tasks on a safe and dependable basis over a defined period of time.

Cognitive Functional Assessment

Cognitive challenges can result from a brain injury, secondary to mental health concerns and  neurological conditions. A cognitive functional assessment is an evidence based evaluation that objectively measures one’s functional abilities to perform the physical and cognitive demands of one’s life including self-care, productivity and leisure. This is a comprehensive assessment that uses measurable, objective testing methods to obtain an accurate representation of cognitive abilities including attention, planning, memory, problem solving, and executive function.

Job Demands Analysis/Functional Job Description is an objective measurement of the essential job demands to determine an employee’s ability to perform his job.

Job-Site Analysis/Ergonomic Assessment is an assessment of the workplace design to determine ways to reduce cumulative stress and strain on the human body so that work can be performed safely and efficiently.

Vocational Assessment is a comprehensive assessment tool used to determine an individual’s employability status based upon their current (adjusted) occupational profile. 

Cost of Future Care Assessment/Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized, concise plan for current and future needs, including associated costs for individuals who have experienced catastrophic injury or have chronic health care needs. Often counselling therapy is required.

Home Assessment is an assessment that determines the individual's ability to perform the physical demands of homemaking, home maintenance and childcare by assessing the environment for physical barriers that may impede function.

Functional Home Maintenance Evaluation is an objective report that confirms, as per the client’s FCE and /or medical reports, along with a client’s intake, his/her future household replacement needs. Assistive devices may be recommended here, as the client’s age, pre-household activity, weather status and size of home/yard, etc., are all relevant in determining a conclusion.
